ASAP Case Challenge Volume 2
Special Populations, Lab Pitfalls, and When to Step Back
Work through 72 new case-based questions on harder clinical situations: patients who are pregnant, frail, young, or athletes; lab results that can mislead; and decisions about when to scale back treatment or monitoring. Cases cover hypercalcemia of malignancy, tumor-induced osteomalacia, hook effect and biotin interference, diabetes technology in pregnancy and type 2 diabetes, MACS management, and radioiodine-refractory thyroid cancer.
Course overview
Volume 2 moves beyond routine care. Cases focus on patients who need a different approach, on lab results that can mislead, and on when to scale back treatment or monitoring. Examples include stopping imaging for a stable pituitary lesion, taking a bisphosphonate holiday, monitoring radioiodine-refractory disease, and continuing an SGLT2 inhibitor as kidney function declines.
Tracks you’ll explore
- Adrenal & Pituitary: Screening adults with hypertension for primary aldosteronism; surgery versus monitoring for MACS; confirming HPA axis recovery and interpreting ACTH stimulation in patients using inhaled steroids; hook effect in a pituitary macroincidentaloma; scaling back MRI surveillance; telling true Cushing syndrome from pseudo-Cushing states by clinical features; and second-line therapy for acromegaly.
- Calcium & Bone: Hypercalcemia of malignancy; tumor-induced osteomalacia; pseudohypoparathyroidism in children; hypophosphatasia and asfotase alfa; switching to romosozumab or teriparatide; bisphosphonate holidays; denosumab in stage 4 CKD; low bone density in an elite athlete; normocalcemic hyperparathyroidism with negative imaging; and vitamin D in healthy older adults.
- Diabetes & Hypoglycemia: Technology for nocturnal hypoglycemia; CGM in pregnancy; automated insulin delivery in type 2 diabetes; SGLT2 inhibitors in advanced CKD, frailty, and HFrEF; GLP-1 receptor agonists for cardiovascular and kidney protection; obesity therapy in type 1 diabetes; diabetes self-management education; and screening for autonomic neuropathy.
- Lipids, Obesity & Nutrition: Preventing pancreatitis from high triglycerides; multifactorial versus familial chylomicronemia; upfront statin plus ezetimibe after an acute coronary syndrome; sleeve gastrectomy versus gastric bypass; pregnancy after duodenal switch; ultra-processed foods, sweetened beverages, and sitting time; baseline assessment before obesity treatment; and phosphate monitoring with ferric carboxymaltose.
- Reproduction: Diagnosing premature ovarian insufficiency; glucose testing in PCOS and early pregnancy; macroprolactin, hook effect, and biotin interference in prolactin testing; lipid changes on testosterone therapy; and persistent menses, hair loss, and lower-cost estradiol in gender-affirming care.
- Thyroid: Monitoring radioiodine-refractory disease; prophylactic central neck dissection; completion thyroidectomy after lobectomy; TSH targets and suppression; managing recurrence; active surveillance outcomes; radioactive iodine for toxic multinodular goiter; GLP-1 receptor agonists before thyroid surgery; teprotumumab hearing effects; and subclinical hypothyroidism in older adults.

After completing this course, learners should be better able to:
- Adapt endocrine evaluation and treatment for pregnancy, older or frail adults, children, athletes, and patients with advanced CKD.
- Recognize lab interference and testing pitfalls, including hook effect, macroprolactin, biotin, and glucocorticoid exposure, and choose appropriate confirmatory testing.
- Identify when treatment, imaging, or surveillance can safely be scaled back.
- Manage less common endocrine conditions, including tumor-induced osteomalacia, hypophosphatasia, pseudohypoparathyroidism, and chylomicronemia syndromes.
This program is intended for clinical endocrinologists.
