ASAP Case Challenge Volume 1

ASAP Case Challenge Volume 1

Confirm the Diagnosis. Choose the Right Therapy.

Work through 72 case-based questions focused on two core skills: confirming an endocrine diagnosis with the right test and choosing the right therapy. Cases cover primary aldosteronism and adrenal venous sampling, osteoporosis treatment sequencing, GLP-1 receptor agonists and SGLT2 inhibitors across comorbidities, hyperprolactinemia, and surgery and TSH targets in thyroid cancer.

Course overview

Volume 1 focuses on the decisions at the start of care: which test confirms the diagnosis, and which therapy best fits the patient in front of you. Cases ask you to choose a confirmatory test, match a treatment to a patient’s comorbidities, and weigh how strong the evidence behind a recommendation is.

Tracks you’ll explore

  • Adrenal & Pituitary: Adrenal venous sampling and MRA dosing in primary aldosteronism; confirming ACTH-independent cortisol secretion; imaging risk stratification of adrenal masses; methoxytyramine in SDHB carriers; the desmopressin test for pseudo-Cushing states; oral octreotide monitoring; and MEN1 and pituitary microadenoma surveillance.
  • Calcium & Bone: Treatment choices in glucocorticoid-induced osteoporosis and after a recent fracture; sequential therapy after denosumab; atypical femoral fracture; parathyroidectomy and normocalcemic hyperparathyroidism; hypoparathyroidism after thyroidectomy and PTH replacement; CYP24A1-related hypercalcemia in pregnancy; and genetic evaluation of early-onset osteoporosis.
  • Diabetes & Hypoglycemia: Choosing GLP-1 receptor agonists and SGLT2 inhibitors in MASH, HFpEF, CKD, type 1 diabetes, and gallbladder disease; interpreting CGM time in range; automated insulin delivery in type 1 diabetes; peripheral artery disease screening with noncompressible vessels; and sexual health in men with diabetes.
  • Lipids, Obesity & Nutrition: apoB and LDL-C discordance; bempedoic acid; continuing semaglutide and managing its tolerability; distinguishing familial from multifactorial chylomicronemia; choosing a bariatric procedure; pregnancy after gastric bypass; intermittent fasting, sweeteners, and active video games; and diabetic gustatory sweating.
  • Reproduction: Functional hypothalamic amenorrhea; selecting candidates for menopausal hormone therapy; monitoring testosterone therapy; long-term monitoring during feminizing hormone therapy; testosterone cost and hair loss in transgender men; and separating drug-induced hyperprolactinemia from prolactinoma.
  • Thyroid: Lobectomy versus total thyroidectomy and how to read the evidence behind that choice; quality of life after thyroid surgery; TSH targets and the risks of suppression; 2025 ATA risk stratification; active surveillance for microcarcinoma; combined T4 and T3 therapy; and subclinical hypothyroidism in heart failure.

Learning Objectives

After completing this course, learners should be better able to:

  • Select appropriate confirmatory and second-line tests for adrenal, pituitary, parathyroid, and lipid disorders.
  • Choose diabetes, obesity, and osteoporosis therapies that match a patient’s comorbidities and fracture or cardiovascular risk.
  • Apply the 2025 ATA risk stratification system and evidence on the extent of surgery to thyroid cancer management.
  • Critically appraise the evidence behind common endocrine treatment recommendations.
Target Audience

This program is intended for clinical endocrinologists.
